Disclaimer / Emergency Consent / Refund Policy

 

Disclaimer:

1. I, the undersigned, being the father, mother, or guardian of the below named child, an infant under the age of 19 years, hereby consent to the Participant being a member of the above mentioned Chiefs Development Group or Chilliwack Sports Entertainment Program, and participating in the Chiefs Development Group or Chilliwack Sports Entertainment Program activities and, on behalf of the said Participant, Hereby assume all risks of personal injury or damage, which may result from participation in any and all activities of this Chiefs Development Group or Chilliwack Sports Entertainment Program.

2. The undersigned on behalf of the participants hereby waives any claims to which he/she may become entitled for injury or damage, and release the Chiefs Development Group or Chilliwack Sports Entertainment Program, all of the Chiefs Development Group or Chilliwack Sports Entertainment officials, officers, directors, staff, owners, organizers, representatives, their agents or employees of any ice facility used by this Chiefs Development Group or Chilliwack Sports Entertainment Program, liable for injury or damage which the Participant may sustain while participating in any Chiefs Development Group or Chilliwack Sports Entertainment Program activity.

3. The undersigned acknowledges on behalf of the Participant that ice skating and the sport of ice hockey have physical dangers which may result in serious injury or death. In the event of emergency, I hereby give my permission to administer any required medical procedures to the Participant.

4. The Participant is advised to carry medical insurance.

5. The undersigned on behalf of the Participant certifies that the Participant has no know medical condition which would prohibit him/her from the playing the sport of ice hockey, and the states that the Participant is in proper physical condition to play the said sport and that he/she is the age of majority.

6. The undersigned will see to it that the Participant wear specified equipment set out in the Chiefs Development Group or Chilliwack Sports Entertainment Program rules.

7. The undersigned will see to it that the Participant will act in a responsible manner in all Chiefs Development Group or Chilliwack Sports Entertainment Program activities.

8. The undersigned agrees to reimburse the Chiefs Development Group or Chilliwack Sports Entertainment program in full within five days of notice, for the cost of any property damage for which the Participant is held responsible by the Chiefs Development Group or Chilliwack Sports Entertainment staff or owners.

9. In the event that the above mentioned activity is ice hockey, the Chiefs Development Group Program Manager has final authority as to what divisions the teams and players will participate.

10. The undersigned on behalf of the participant understands and agrees that the Participant may be expelled from the Chiefs Development Group or Chilliwack Sports Entertainment Program for any of the following:

• financial delinquency
• failure to abide by all Chiefs Development Group Program Rules
• falsification of registration information

 Refund Policies:

The Chilliwack Coliseum does not issue refunds for many of our programs.  However, we do issue credits that can be used for other programs or lessons at a later date. Should a refund be issued the following policy below will be followed.

Learn to Skate – Fall/Winter/Spring

• Cancelled registrants due to illness/injury (with Doctor’s note) prior to the close of registration will receive a refund.
• No refunds will be issued after the close of registration.
• All refunds are subject to a $25.00 administration fee.

Chilliwack Adult Hockey League – Adult

• Cancelled registrations are subject to a 50% administration fee
• No refunds will be issued after the close of registration

Chilliwack Coliseum – Summer Camps and Chilliwack Chiefs Programming

• Cancelled registrants due to illness/injury (with Doctor’s note) prior to the close of registration will receive a refund less a $25 administration fee.
• All refunds prior to the close of registration are subject to a $75 administration fee.
• No refunds will be issued after the close of registration
• If a refund is granted after the close of registration a $100 administration fee will be levied.

 

Emergency Consent Information:

Consent Form to Be Completed at Time of Registration or Before First Session

It is Chiefs Development Group policy to notify a parent when a child is ill or needs medical attention. Occasionally a situation may arise where Chiefs Development Group cannot contact parents and immediate help (i.e. medical attention) must be sought for your child. Please sign the consent below granting Chiefs Development Group the authority to take appropriate action on behalf of your child should such a situation arise. We will take this signed consent form with us to the emergency centre as proof of our authority. I, the undersigned, authorize the staff or person(s) in charge of the above named child to call a physician, take my child to the nearest emergency centre, or summon an ambulance for emergency medical aid, should, in the opinion of the person(s) in attendance, feel such services are required and I cannot be contacted by phone. I agree that any cost incurred for such services shall be my sole responsibility. If such an emergency should arise, I shall be notified as soon as possible.

I hereby give consent for my child to play in the Youth Division the Chilliwack Coliseum Hockey League for the current season and to receive any emergency medical aid as necessary. I assume all risks and hazards incidental to such participation, including transportation to and from the activities if required and do hereby waive, release and absolve indemnity and agree to hold harmless the Chilliwack Coliseum Hockey League, the organizers, sponsors, supervisors, participants and persons transporting my child to or from activities, from any claims arising out of an injury to my child, except to the extent and in the amount covered by accident or liability insurance.
